- If a Scott or Carver County resident, call 2-1-1, and request for Scott-Carver County FHPAP prescreen - If a Dakota County resident, call the CAP Agency's Dakota County FHPAP Intake Line

Eligibility

Adults who are homeless or at risk of becoming homeless and are: - Residents of Carver and Scott counties - Residents of Dakota County except the cities of Burnsville, Eagan, Farmington, Lakeville, and Rosemount - Income must be at or below 200% of federal poverty guidelines
