Services include: - Accompaniment: Supports individuals during medical or other appointments - Advocacy: Provides clarity on healthcare and service options and advocates individuals needs, or the needs of their loved one - Assessment: Assists with understanding each individual's medical, social, psychological, and spiritual needs - Caregiver Consultation and Support: Offers guidance and support to caregivers - Insurance and Benefit Assistance: Provides help with insurance, Medicare, and benefit forms - Mediation: Facilitates discussions and advocates for the individual's needs in decision-making processes - Referrals and Coordination of Community Resources: Connects individuals with relevant community service options - Relocation Support and Services: Assists with transitions and moving support between healthcare settings - Training and Recommendations: Offers training and recommendations for service providers based on individual support plans - Tailored Care Planning: Creates individualized care plans that account for unique circumstances, treatment goals, and preferences - Technology Innovations: Explores and implements technology to support independence and health
