HOPE Program - Hennepin Healthcare
The HOPE Program treats diagnoses including schizophrenia, schizoaffective disorder, and schizophreniform disorder. Treatment in the HOPE Program is provided by a team of mental health professionals who focus on helping people work toward personal goals and recovery. Once accepted into the program, services include: - Family therapy and education focused on a variety of recovery-oriented topics - Family peer support to promote resilience and self-care, and provide resources for family members and loved ones - Individual therapy focused on learning and practicing new skills for resiliency and recovery - Individualized psychiatric medication evaluation and management services - Nursing to assist with medication questions/concerns and education on healthy living - Psychiatric case management to assist with accessing additional community resources - Peer support focused around recovery goals, resiliency, and self-care strategies for the patient - Psychological assessments for measurement-based care both to individualize treatment and track progress toward goals - Supported employment and education to support maintaining and/or resuming work and school activities

Eligibility
Open to all individuals who have experienced the onset of psychotic symptoms within the last two years, have less than 12 cumulative months on antipsychotic medications, and have diagnoses of schizophrenia, schizoaffective disorder, or schizophreniform disorder
