Eligibility
- Older adults and adults and adolescents with developmental, emotional or physical disabilities
- Serves Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties
- Must be enrolled in one of Minnesota’s health care programs or Medical Assistance or waiver program. Some people on Minnesota Care are also eligible.
- Must have had a previous assessment by the county, tribal nation or managed care organization that confirms eligibility

Service area
Provides personal care assistance services for older adults and people with disabilities in their homes or in a community setting to help them remain independent in the community, including:
- Activities of Daily Living (ADLs): Activities a person needs to carry out on a daily basis to remain healthy and safe including dressing, grooming, bathing, eating, positioning, transfers, and mobility.
- Instrumental Activities of Daily Living (IADLs): Activities a person needs to carry out on a regular basis to remain independent including shopping, paying bills, and meal preparation.
- Health-related procedures and tasks: Tasks such as supporting a person with self-administered medications or help with range of motion exercises.
- Observation and redirection of behaviors: Monitoring a person’s behaviors and redirecting them to more positive behaviors when needed.
