Scholarship Program - Patient Advocate Foundation

Two scholarships are offered to national undergraduates and graduates. Scholarship amount and distribution depend on eligibility requirements. National Undergraduate Scholarship: - $3,000 annually up to four consecutive years until the student receives a maximum of $12,000 or graduates with a bachelor's degree, whichever comes first - Payable in equal amounts (per semester/trimester/quarter) directly to the college, university, or vocational-technical school for the purpose of defraying tuition and other fee costs (books are not to be included) Graduate Scholarship: - $3,000 per school year for up to two consecutive years until the student receives a maximum of $6,000 or graduates with a Graduate or Ph.D. degree, whichever comes first - Payable in equal amounts (per semester/trimester/quarter) directly to the college or university for the purpose of defraying tuition and other fee costs (books are not to be included)

Individuals that are legal residents of the United States, under the age of 35, who have been diagnosed with or treated for cancer and/or a chronic/life-threatening disease within the past 10 years; statewide
