Serves Leech Lake Reservation, Beltrami, Cass, Hubbard and Itasca counties.
- Pregnant or postpartum women
- Infants and children up to age 5
- Based on income (18% of federal poverty guidelines) for pregnant, postpartum and breast feeding women.
