A nurse home visiting program to work toward the improvement of:
  • Family economic self-sufficiency
  • Maternal and child health well being
  • Prevention of child abuse or maltreatment
  • Prevention of childhood injuries and reduction of emergency department visits
  • Reduction in a crime or domestic violence
  • Referrals to other community resources and supports
  • School readiness
NOTE: Nurse-Family Partnership works by having specially trained nurses regularly visit young, first-time moms-to-be, starting early in the pregnancy, continuing through the child's second birthday
