Provides assistance to individuals in the home or has in-home safety concerns to help with daily living activities and personal care. Services include:
- Additional support and education to care for a high-risk newborn, or patient of any age, whose needs are great
- Assistance transitioning from hospital to home
- Bathing, housekeeping, shopping, or meal preparation
- Dressing changes, wound care, or overall assessments
- Home health care
- Hospice care
- Medication management
- Occupational therapy to address access and safety concerns in the home
- Physical therapy
- Speech therapy when needed for recovery from a stroke
