- Home visits to provide antepartum, postpartum, breast feeding, family planning, infant care, child growth and development assessments, education, promotion of parenting, family planning information and prenatal and childbirth education - Provides services to children with special needs, including skilled nursing visits, waiver case management, personal care attendant assessment and screening for Early Childhood Special Education services - Follow Along Program provides information regarding growth and development and screen for need for referral to others services, including medical and Early Childhood Special Education - Family Service Aides provide limited homemaking, support and education to pregnant women and new moms and infants, and parenting support to families - Education related to injury prevention including home safety - Community resource information and referral - Nutrition education
