  • Sliding Scale/Reduced Fee
  • Home visits to provide antepartum, postpartum, breastfeeding, family planning, infant care, child growth and development assessments, education, promotion of parenting, family planning information, and prenatal and childbirth education
  • Provides services to children with special needs, including skilled nursing visits, waiver case management, personal care attendant assessment, and screening for Early Childhood Special Education services
  • Follow Along Program provides information regarding growth and development and screens for the need for referral to other services, including medical and Early Childhood Special Education
  • Family Service Aides provide limited homemaking, support, and education to pregnant women and new moms and infants, and parenting support to families
  • Education related to injury prevention, including home safety
  • Community resource information and referral
  • Nutrition education
