Application process

Call for an appointment and ask to speak to an intake coordinator or visit the website to register for an initial appointment. Those registering via the website will be contacted to schedule an appointment.

Fee

Vary according to services provided. Supervised intern services are available periodically at a reduced rate. Call for information.

Required documents

Once an appointment has been scheduled, new clients must complete registration paperwork, which includes medical history, prior to their initial appointment. Those using insurance, including Medical Assistance, should bring their insurance card and photo ID with them to the first appointment.
