Provides two group home residence locations, Brooklyn Park and Minneapolis, for elderly and disabled adults. Services include 24 hour customized living, private RN, LPN, TMA, PCAs, medication administration, 3 meals per day, arrange for transportation, assistance accessing community, laundry and housekeeping, continence care, and transfer assistance. Provides in-home personal care assistance, including assistance with activities of daily living, light duty cleaning, cooking, assistance bathing, grooming, personal hygiene, transfer assistance, continence care, self-medication set-up, medication management, complex medical care, and other health-related procedures.
