Provides services in 12 adult foster care homes throughout Minnesota. Each home offers an individualized, rehabilitative approach to treatment and support for people with mental illness. Services offered include medication management, medication education, medication supervision, physical health management, independent living skills building, transportation, trained staff supervision (24 hours at most homes), groups, individual counseling, socialization opportunities, and much more, in a home environment.
