Homeless to Housing - Hennepin County Human Services - Social Services

Eligibility
Single adults, families, youth, and veterans who are ages 18 years and older who:
  • Are not already working with a housing specific case manager
  • Currently staying in a shelter, sleeping outside, or in a vehicle within Hennepin County
Required documents
Application/registration form birth certificate for families with children photo ID proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.) shelter verification verification of the last 30 days of income and assets including more supporting documents can help process applications quickly

Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. The primary focus of the Homeless to Housing team is to fill system gaps by offering case management to people who otherwise lack support and access to resources by helping them secure permanent housing. The Homeless to Housing teams can help single adults, families, youth, and veterans who are staying in shelter or those living unsheltered with the following: 1. Referral 2. Intake 3. Goal planning 4. Barrier busting 5. Housing identification and move in 6. Stabilization and referral to long-term support
