Provides the following services: - Care coordination and consultation by licensed nurses and social workers - Assessment of needs - Facilitation of family meetings and professional care conferences - Information and referral to services - Liason for family members with health care providers - Accompaniment to medical appointments - Review of current Medicare, Medicaid and private insurance coverage providing explanation of benefits and assistance with organizing and submitting health insurance claims and/or hospital bills - Caregiver coaching - On call only services which provides 24/7 support and guidance in the event of a crisis
