Targeted Case Management: Provides person-centered case management to Ramsey County residents diagnosed with a Serious and Persistent Mental Illness (SPMI). Assist individuals in identifying needs, developing goals, and linking to services such as mental health care, substance use treatment, medical care, housing resources, vocational services, social supports, legal and educational services, and community-based supports. Community Care Services Team: Specializes in supporting individuals with mental health and substance use disorders who are transitioning back into the community after extended or repeated stays in hospitals or other treatment facilities. Coordinates care across providers and systems and helps with assessment, service planning, treatment plan development, implementation, and ongoing evaluation. The team collaborates with county agencies, hospitals, mental health providers, primary care, and community services to promote stability and prevent rehospitalization.
