Description

Provides comprehensive mental health services designed to support adults in their journey to improved well-being. Services offered: - Assertive Community Treatment (ACT): Provides personalized support such as psychiatry, therapy, psychiatric nursing, treatment planning, goal development, vocational support, co-occurring disorders interventions, peer support, psychoeducation for individuals served by their families, and 24 hours/7 days a week assistance. - Adult Targeted Case Management (ATCM): Provides individualized and compassionate support for adults facing mental health challenges and need help navigating essential services. Case managers work closely with clients to foster long-term well-being and independence by identifying specific needs, connecting them to valuable community resources, and supporting them reach their personal and wellness goals. - Children's Case Management Services (CTCM): A voluntary service aimed at providing holistic support for children facing emotional challenges. Case managers work closely to create customized plans that address medical, social, educational, and vocational needs. Additional services include coordination of support services, assessment of mental health, coping skills training, parent education, monitoring needed services, and advocacy. - Sage Enrichment Center: A community-based meeting facility for individuals dealing with mental illness. Offers recreational and leisure activities.
