Co-pay may be established based on income level and family size; sliding scale

Required documents

Application/registration form; picture ID; Form DHS-316B for each absent parent not living in the home; proof of address; proof of citizenship/passport; proof of employment and work schedule from the last two months; pay stubs from the last 30 days; Social Security card/number; state ID

Eligibility

Families who meet at or below 175% of the Federal Poverty Guideline and are residents of Freeborn County

The Department administers the various cash and medical assistance programs as well as child support; is a member of the Blue Plus, UCare, and SCHA delivering the MA programs; operates a licensed Mental Health Center and Crime Victims' Crisis Center; provides child and vulnerable adult protective services; services to persons with developmental disabilities and chemical dependency; in-home family counseling; provides adolescent (SED) day treatment jointly with the Albert Lea School District and children's mental health services.
