A web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports in Minnesota. The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools: Developmental Disability Screening Long-term Care Consultation Assessment Personal Care Assistance Assessment Home Care Nursing Assessment (to be included in a future enhancement) The assessment determines eligibility for publicly funded programs and services including: Alternative Care Program Brain Injury Waiver Community Alternative Care Waiver Community Access for Disability Inclusion Waiver Consumer Support Grant Developmental Disability Waiver Elderly Waiver Essential Community Supports for Seniors Moving Home Minnesota Personal care assistance Case management Semi-Independent living services
