Description

The assessment helps determine public programs that might pay for an individual's long-term services and supports or help them learn about other options if they don't qualify for publicly funded programs. It is required to determine eligibility for: Community First Services and Supports - CFSS (formerly known as Personal Care Assistance - PCA) Home and Community-Based Waivers based on age: - Individuals Over 65 Years Old: Alternative Care (AC); Elderly Waiver (EW) - Individuals Under 65 Years Old: Brain Injury Waiver (BI); Community Access for Disability Inclusion Waiver (CADI); Community Alternative Care Waiver (CAC) Intellectual and Developmental Disability (IDD) Services (also known as Rue 185 Case Management) Family, friends, or others can attend the assessment. The assessment is in person at the individual's home or at a place of their choice, and it lasts about two hours.
