MnCHOICES - Kandiyohi County Public Health and Human Services
A comprehensive web-based application that integrates assessment and support planning for people who need long-term services and supports in Minnesota. The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools: - Developmental Disability Screening - Long-term Care Consultation Assessment - Personal Care Assistance Assessment - Home Care Nursing Assessment (to be included in a future enhancement) The assessment determines eligibility for publicly funded programs and services, including: - Alternative Care Program - Brain Injury Waiver - Community Alternative Care Waiver - Community Alternatives for Disabled Individuals Waiver - Developmental Disability Waiver - Elderly Waiver - Case management for people with developmental disabilities - Semi-Independent Living Services NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services
