Housing Support - Kandiyohi County Public Health and Human Services

Provides room and board for low-income seniors and adults with disabilities with the goal of preventing institutionalization and homelessness. Individuals who do not qualify for home and community-based waiver programs may be able to receive supplemental service funding to provide other services, including but not limited to: - Arranging for appointments, meetings, and medical and social services - Assistance with transportation - Medication reminders

Fee

The individual's housing provider receives payments to pay for the individual's food, household supplies, rent, utilities, and other necessities. Depending on the individual's income, they may have to pay a portion of the Housing Support housing rate directly to the housing provider.

Eligibility

Residents of Kandiyohi County must meet a combination of eligibility requirements set by the Supplemental Security Income program or General Assistance program to qualify for assistance. There are also income and asset limits. Generally, the program serves: - Adults ages 65 and older - People younger than age 65 who have a condition that limits their self-sufficiency; examples include a physical or mental health disability, visual impairment, or substance abuse
