Provides home health care services to meet the needs of the individual residing in their home. Home care plans are established to provide services such as:

Ambulatory and transfer services within the home for individuals who need assistance

Assistance with personal care for bathing, dressing, and grooming

Homemaking services to help with sweeping, mopping, dishes, cleaning, changing linens, and laundry assistance

Home safety assessments

In-home respite care

Medication management

Nursing evaluations and nursing care

Prepare meals

Referrals to appropriate community resources
