Outpatient Substance Abuse Treatment - CREATE, Inc.

Substance use disorder and mental health services designed to help individuals define the problems substance use has caused and make goals for a chemically healthy future. Mixes individual and group therapy depending upon individual need. Includes programming for opioid users. Programs offered are low, medium, and intensive depending on assessment recommendation. All services are offered in-person or virtually through a Telehealth link.

- 9 am - 12 noon and 6 pm - 9 pm Monday - Thursday - Meetings will occur between 1 - 3 days weekly and be based on individual's needs.

- Any adult interested in or in need of substance abuse services; must have substance use disorder assessment within the past 6 months that qualifies for outpatient treatment - Primarily serves Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties

Services provided are substance use disorder assessments (both Rule 25 and Comprehensive assessments), intensive out-patient treatment with mental health services, opioid track program, low intensity out-patient treatment, recovery maintenance programs, individual counseling and educational programs.
