- Individuals who are pregnant, recently delivered a baby, or parenting a child under the age of 4 1/2 - Families with children who have special pediatric services are eligible such as NICU follow-up, Pediatric asthma, chronically ill children, and children with special needs due to a lead poisoning - Residents of Carlton County
Application/referral form birth certificate for families with children photo ID proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.) shelter verification verification of the last 30 days of income and assets any other supporting documents can help process applications quickly

Registered Nurses provide a variety of programs to serve families. These programs include family home visiting during pregnancy and after the birth of your child, breastfeeding support, developmental screening for your child, car seat distribution, and more.
