- Currently a homeless parent or parents with minor children - Be disabled or care for a disabled child - Capable of providing written verification of custody, disability and homeless status - Primarily serves Chisago, Isanti, Kanabec, Mille Lacs, Pine, Crow Wing and Sherburne counties

Description

Permanent supportive housing providing case management to encourage stability to homeless families who have a family member with a chronic disability including intake assessment, creating a family plan, referral and advocacy, budget debt reduction, life skills, parenting support, job search, housing search, goal planning and tracking.
