The program provides education, serves as a community and family resource, and contributes extensively to the advancement of HIV-related knowledge. - Case management: Social workers can assist with housing needs, transportation assistance, applying for financial assistance, and clarifying insurance questions - Chemical health assessments - Free, confidential HIV testing for partners of patients - HIV education materials - Nutritional assessments - Medication management - Primary medical care - Psychotherapy services
