Assists older adults and caregivers maintain their health, wellbeing, and independent. Seniors, older adults, adults with mental, physical and chronic health conditions and their caregivers have access to a variety of healthy aging and caregiving services, support and resources. Services include: - Caregiver respite - Consultation - Peer support groups

Application process

Call to schedule a tour and intake meeting with a social worker. During this time, paperwork to enroll participant will be completed and transportation will arrange if needed. Call, write, or visit website for general information.

Fee

Fees vary by service and sliding scale. Call for fee information. Extended hours from 7 am - 8 am and 4 pm - 5 pm require an extra charge of $10 each.

Required documents

Application needs to be completed before starting. Medical history and physical is a requirement prior to starting or within 30 days of start date.
