Financial Assistance Program - North Memorial Clinics - Brooklyn Park

Financial Assistance Program: Provides financial assistance (charity care) to low-income, uninsured, or underinsured individuals who need help paying for all or part of their medical care. Patients are eligible when their family income is at or below 300% of the Federal Poverty Level (FPL) income guidelines. Patients must apply for financial assistance within 240 days after the date of their first post-services billing statement. Applicants are notified of financial assistance eligibility in writing within 30 days of receipt of the application. Uninsured Discount: Offers a discount to customers who are not eligible for financial assistance and income is less than $125,000 Senior Partners Care: Partner with Senior Community Services to waive clinic co-insurance and deductibles for customers who are members of Senior Partners Care AblePay: Program allows patients to save up to 13% on deductibles, coinsurance, and copays at no cost to the patient Extended Payment Plan through ClearBalance: Provides an affordable patient financing program with flexible repayment terms to help consumers pay out-of-pocket hospital expenses, including deductibles and insurance co-payments
