In-Home Assistance - Como Park Falcon Heights Living At Home Block Nurse Program
The Living at Home Block Nurse Program provides support services to elders and their caregivers so they may continue living in their home as long as possible. This support is available through volunteers and four part-time staff. No-cost services include: - Application assistance - Blood pressure clinics - Care management - Friendly visits (phone calls only until further notice) - Home safety checks - In-home fall prevention coaching - Respite care - Service coordination - Spring and fall yard clean-up - Transportation services Services provided through private pay or paid by insurance/Medicare: - Homemaking services - Chores support - In-home foot care - Navigation assistance

Application process
Call; email; visit website; walk in; or write for information. Seniors do not need to use home health care in order to qualify for volunteer services. A one-week notice is required for transportation services.
Fee
- Call for information. Volunteer assistance is without charge. A home visit by a nurse to develop a care plan and assess whether the senior is eligible for county services is offered at no cost (though donations are accepted). - In-home foot care provided for $40. Homemaker services provided for $25 per hour for a minimum of 90 minutes. - Nursing and home health aide services billed to insurance or private pay. Private pay services are less than market rate. Donations accepted.
Eligibility
Persons 65 years and older living in the Como Park area of Saint Paul and in Falcon Heights. Roselawn/Snelling/Railroad Tracks on the west half and Larpenter/Dale/Maryland on the east half.
