Application process
Call or walk-in to a drop-in center or shelter to connect with a street outreach worker for a Coordinated Entry Assessment - A referral is required through the Hennepin County Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment, will not be able to access Coordinated Entry.

Eligibility
Individuals ages 18 -24 who are homeless in Hennepin County and meet the following criteria: - Be verified homeless in Hennepin County on the day of intake with a letter from an outreach worker indicating the person is living in a place not meant for human habitation or a letter from an emergency shelter - Be capable of living independently - Be willing to participate in RRH Program services
