Offers four residential service options that support individuals with disabilities in community-based settings:
  • hoMEbase Apartment Program: Provides a balance of independence and support for individuals living in their own or shared apartments.
  • Intermediate Care Facilities (ICF-MR): Delivers 24-hour awake supervision in medically modeled group homes for individuals with higher support needs.
  • Semi-Independent Living Services (SILS): Assists individuals transitioning into the community, offering an average of 5-6 hours of weekly support to promote independent living.
  • Waiver-Funded Group Homes: Provides 24-hour supervised residential care within community homes for individuals with disabilities.
