Open to all seniors living in St. Anthony Park, all of Lauderdale, and Falcon Heights west of Cleveland Avenue. Also provides support to caregivers living in this area even if the cared-for person lives elsewhere.

Application process
- Call to schedule an in-home visit which usually take one to two hours. Services provided based on client's request and assessment results. Services generally are available starting seven days after assessment, although time can vary depending on services needed.
- Transportation requests must be made at one week in advance.
Fees
Volunteer services available without fee, although donations are accepted. Nursing and handyman has associated fees. Subsidies available for foot care based on income status.

- Case management and referrals for programs such as Meals on Wheels, social services
- Door-to-door transportation mostly for health related appointments or for hair salons/barbershop appointments. Clients must be able to transfer into the car on their own. Escort services may also be available, but these requests must be made far in advance.

- Senior exercise classes are offered daily; transportation available.

- Support for caregivers of seniors who live in the service area, and for caregivers who live in the service area even if the cared-for senior lives elsewhere, including out of state.
