Application process

- Call or walk in; must attend an interview and complete enrollment forms - Complete one-on-one interview with recruiter and Integrated Care Team - If incarcerated, work with case manager to complete forms and phone interview - Supportive Housing: A referral is required through the Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment will not be able to access Coordinated Entry.

Eligibility

- Men with a history of incarceration and homelessness - Ex-offenders with a history of arson or sex offenses are not eligible - Men on probation or parole in Hennepin and Ramsey counties - Primarily serves residents of Hennepin and Ramsey counties
