Residents of Polk County who are:
  • Parents or alleged parents of a minor child
  • Parents of a minor child and if one parent does not live with the child
  • Parents who pay court-ordered child support
  • Have court-ordered, physical custody of a minor child
  • Receiving public assistance for a minor child who lives in their home
