Application process
  • Call; appointments are preferred; it is recommended to call early in the morning if a same-day appointment is needed
  • Walk in: Weekly STI Testing only:
  • Fremont Clinic: 1 pm - 4 pm Tuesday, Thursday, and Friday
  • Central Clinic: 1 pm - 4 pm Wednesday
  • For health insurance questions, call the billing department. Patient advocates will help those eligible to enroll in Medical Assistance and other public health insurance programs.

  • Individuals with no insurance who verify income will be put on a sliding fee scale according to family size and income. No one is turned away due to inability to pay.
  • NOTE: Minimum payment is $20
