• Will maintain anonymity and confidentiality

• Will require name, address and Social Security number in order to provide ANY services.

• If a person doesn't have a Social Security card, a driver's license may be acceptable. Also see "Transient Policy"

Coordinated Entry Services (CES): The Housing Advocacy Services Department participates in the Southeast Minnesota River Valleys Continuum of Care Coordinated Entry Services (CES) for housing and support services for households experiencing homelessness Prevention Assistance: When funding is available, individuals and families will be referred to the Housing Advocacy Department for follow up on Prevention Assistance. Funds will vary on a monthly basis. The Prevention Assistance fund assists families and individuals facing eviction with back-rent financial assistance and landlord-tenant negotiation assistance. Case management includes services such as transportation assistance, information/referral, and landlord tenant law training with the goal of achieving housing stability moving forward. Rapid Re-Housing (RRH): Provides short-term rent assistance and/or deposit for individuals and families with children who are homeless to get into permanent affordable housing where the participant is the leaseholder and chooses their own apartment according to their needs. RRH includes case management services such as life skills training, transportation assistance, information/referral, landlord tenant law training, and utility assistance for up to six months with the goal of obtaining and subsequently maintaining stable housing. Households will be assessed and prioritized using the Coordinated Entry System. Long-Term Assistance: The Permanent Supportive Housing (PSH) Program provides high-barrier households experiencing homelessness with affordable rental housing and supportive services that assist participants with maintaining housing stability, improving overall wellness, engaging in the community and leading successful lives. PSH targets households that need intensive case management services due to challenges such as significant and persistent mental health issues and chemical dependency. Households will be assessed and prioritized using the Coordinated Entry System. Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.

• Outpatient medical care through the V.A.

• Service-connected disability compensation

• "State Soldiers Assistance Program" (SSAP) provides cash assistance in the form of shelter payments (rent and mortgage), utilities, and personal needs grants to veterans who are unable to work as a result of a temporary disability. There are strict income and asset requirements.

Provides financial assistance, homelessness programs, and assessments for supportive housing to households who are at imminent risk or are homeless. Services provided are: Case Management

• Assessing barriers that prevent stable housing

• Provides budgeting, life skills support toward employment and education goals

Coordinated Entry

• Assessments for supportive housing programs and other support services
• Fond Du Lac Property Management provides 24 supportive housing units through coordinated entry
• Fond Du Lac Property Management provides 10 veteran supportive housing units through coordinated entry

Emergency Rental Assistance Program

• Assist with one month's rent and security deposit not to exceed the value of one month's rent. Assistance is not available to households who have received assistance from the program within a 3-year period. Payments are made directly to the landlord.

Homeless Prevention Programs

• Prevention Services: Temporary financial assistance and/or supportive services to help people remain in their current housing situation, relocate, or divert them from entering the homeless system. Financial assistance may help with rental application fees, the first month's rent, past due rent, or utility assistance.
• Rapid-Rehousing: Assists households to quickly exit homelessness and return to permanent housing. Provides one-time rental assistance and supportive services such as case management and housing navigation.

Homeownership Down Payment Assistance

• Assistance under this program may include payment of reasonable loan down payments, closing, and/or costs generally referred to as settlement charges associated with the first mortgage of the home to be used as the family's primary residence. Maximum amount of assistance is $3,000 including down payment, closing, and settlement costs.
• Helps secure affordable homeownership for Fond Du Lac band members

Intake process for programs serving individuals and families who are in search of housing programs or are in need of financial assistance for housing. Family Homeless Prevention and Assistance Program: Helps prevent homelessness by providing one-time assistance with rent or mortgage payment for households until their financial situation can stabilize. Offers case management and follow-up services. Rapid Rehousing Program: Provides information, referral, advocacy, and first month's rent and/or security deposit assistance to individuals experiencing homelessness, doubled up, or homeless households moving into a housing unit. Ongoing rent subsidy may be provided until the household is able to sustain their rental housing.

• Applicant must have spent at least 30% of their available income on housing and utilities in the past 90 days

• Applicant's name must be on the lease or utility bill

• Financial assistance is paid directly to the vendor?

Provides services to help prevent families from becoming homeless, including resource referrals and eviction prevention assistance. Families are required to participate in three months of stabilization case management in order to receive financial support.

Provides a program called the Project for Assistance in Transition from Homelessness (PATH) to connect individuals with housing services and other resources to find safety and housing stability. Services provided through the program:

• Helps individuals apply for economic assistance, medical assistance, and Social Security as needed

• Helps individuals obtain necessary paperwork to secure housing and create an individualized housing plan

• Provides referrals to medical and mental health care, legal assistance, and other essential services

• Works through barriers preventing the individual from finding and keeping housing

• Works with clients to access Coordinated Entry services to determine if they qualify for Project Home or Youngdahl Living programs. Project Home and Youngdahl programs are permanent support housing programs for those dealing with homelessness, unstable living conditions, and need stable housing.

Through a voucher system administered by on-call volunteers from member churches, funds are set up to help those who fall in between the cracks of other assistance. Vouchers are provided for electric service, gas money, gas service, heating fuel, rental deposit assistance, rent payment assistance, and water service assistance. Vouchers do not cover the amount of a total client's bill, the client must pay some of the bill on their own. Volunteers provide spiritual ministry as well as the ministry of meeting the person's basic needs for emergency food and shelter. • Cub Foods gift cards

• Gasoline purchases using Holiday fuel cards

• Rent or mortgage payment

• Home or cell phone bill

• Gas, electric, or water bill

• Garbage bill

Provides information, resources, and support needed to move life forward. These partners help navigate the challenges of Multiple Sclerosis (MS) unique to each individual's situation. In addition to connections to other MS Society services navigators:

• Assess for personalized case management through the Edward M. Dowd Personal Advocate Program

Provide crisis intervention in times of need

• Help provide wellness strategies that can improve quality of life including suggestions on diet, exercise, emotional well-being, and connection to local resources

• Navigate through the complexities of finding healthcare providers, benefits, insurance and access to coverage, and employment

• Offer financial assistance with obtaining durable medical equipment and aids for daily living, emergency financial assistance for help with housing or utility shut off, respite care, chores, home modifications, and more. Will assist individuals with future planning.

In partnership with Hennepin County, Isuroon provides a rent assistance program connected to Operation Metro Surge. Funding is available to renters facing an eviction for any reason, including those affected by immigration enforcement.
