Provides emergency rental assistance with goal-oriented case management to address the immediate housing crisis, prevent homelessness, and promote long-term housing stability by addressing barriers related to homelessness. Supportive services may include goal setting and stability planning, budgeting support, employment search, transportation assistance, childcare and benefits navigation, help identifying healthcare providers, and much more. Provides rapid re-housing services where members of the community open their homes to host a youth or young adult in Anoka County who are experiencing homelessness. Provides case management services to help facilitate transitions, minimize barriers, and help youth and young adults set and achieve their goals.

Intake process for programs serving individuals and families who are in search of housing programs or are in need of financial assistance for housing. Family Homeless Prevention and Assistance Program This program helps prevent homelessness by providing one-time assistance with a rent or mortgage payment for households that need a short-term "bridge" until their financial situation is expected to stabilize. Case management and follow-up services provided. Transitional Housing Program (Rapid Rehousing) This program provides information, referral, advocacy, and first month's rent and/or security deposit assistance to doubled up or homeless households moving into a housing unit. Ongoing rent subsidy may be provided until the household is able to sustain their rental housing.

Food shelf which can be used once in a 30-day period. Families receive food for three - five days. Hygiene items may be available as supplies last. Vouchers for clothing (adults and families), and bedding/linens. Provides Bridging referrals for individuals needing furniture. Transportation: Bus and gas cards are available for individuals who are experiencing homelessness and in a Coordinated Entry program or for individuals who are starting a job but haven't received their first paycheck yet. HeatShare, seasonal services, Family Camp, back to school program, seasonal programs Coordinated Entry assessments for adult families and singles 24 and older in Anoka County Family Homeless Prevention and Assistance Program (FHPAP): Funds are only available to individuals and families facing an eviction. Funding varies month to month and may not be available. NOTE: One phone line is set aside for this program. It stays open until there are more than 80 requests per week. Then it is closes until staff are caught up on intakes. When the intakes are caught up, the line is opened again. Individuals should try the phone number a few times throughout the week. If someone asks, let them know to keep checking Line 5 - it will open again soon. Rapid Rehousing: Provides services for people experiencing homelessness that have found housing. Street Outreach: Provides outreach services such as food and transportation for people experiencing homelessness.

Provides short-term help for low-income households who are homeless or at imminent risk of homelessness. Direct financial assistance may include foreclosure payments, rent payments, rent subsidies, security deposits, first month's rent, utility payments, or other assistance to prevent eviction. Minimal assistance is also available for food, child care, transportation, and work clothing and supplies. The purpose of these programs is to stabilize families in their home or re-house families that are homeless.

Coordinated Entry Services (CES): The Housing Advocacy Services Department participates in the Southeast Minnesota River Valleys Continuum of Care Coordinated Entry Services (CES) for housing and support services for households experiencing homelessness Prevention Assistance: When funding is available, individuals and families will be referred to the Housing Advocacy Department for follow up on Prevention Assistance. Funds will vary on a monthly basis. The Prevention Assistance fund assists families and individuals facing eviction with back-rent financial assistance and landlord-tenant negotiation assistance. Case management includes services such as transportation assistance, information/referral, and landlord tenant law training with the goal of achieving housing stability moving forward. Rapid Re-Housing (RRH): Provides short-term rent assistance and/or deposit for individuals and families with children who are homeless to get into permanent affordable housing where the participant is the leaseholder and chooses their own apartment according to their needs. RRH includes case management services such as life skills training, transportation assistance, information/referral, landlord tenant law training, and utility assistance for up to six months with the goal of obtaining and subsequently maintaining stable housing. Households will be assessed and prioritized using the Coordinated Entry System. Long-Term Assistance: The Permanent Supportive Housing (PSH) Program provides high-barrier households experiencing homelessness with affordable rental housing and supportive services that assist participants with maintaining housing stability, improving overall wellness, engaging in the community and leading successful lives. PSH targets households that need intensive case management services due to challenges such as significant and persistent mental health issues and chemical dependency. Households will be assessed and prioritized using the Coordinated Entry System. Note: Funding comes from a variety of sources including the Family Homeless Prevention and Assistance Program - FHPAP and others.

Provides free and confidential services to those affected by domestic violence, sexual violence, human trafficking/exploitation, and high-risk youth in Lincoln, Lyon, Redwood, and Yellow Medicine counties. Confidential services: - Accompaniment to court - Assistance with petitioning for an Order for Protection or Harassment Restraining Order - Community education - Crisis line - Emergency transportation, and/or assistance - Information, referrals, and resources - Temporary emergency safe housing - Systems coordination - Support group - Youth Programs: Provides youth the ability to teach and learn from their peers about confidence, communication, safety, and life skills. Emergency assistance includes, but is not limited to, the following expenses: - Crime Scene cleanup - Grant funds are used to meet crime victims' emergency needs when available - Purchase and installation of necessary home security devices - Reimbursement of towing and storage fees incurred due to impoundment of a recovered stolen vehicle - Replacement of necessary property that was lost, damaged or stolen as a result of a crime - Transportation to locations related to the victim's needs as a victim, such as medical and criminal justice facilities - Victim reimbursement for reasonable travel and living expenses incurred due to a change in venue Victims of crime have also received funding through the program for items such as repairing a broken door, installing new locks, and replacing clothing or bedding taken as evidence. Family Homeless Prevention and Assistance Program - FHPAP may be available through pre-screening.

Regional Coordinated Entry to Housing Programs agency The family development program provides services that help persons with emergencies and crises, self-sufficiency, shelter and housing, homeless services, and several special projects. Services may include: - Case management - Emergency housing - Foreclosure prevention assistance - Permanent supportive housing - Rent/deposit assistance - Transitional housing

Offers person-centered support services to individuals living with a disability or mental health condition. Services provided: - Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless - Develop life skills including self-sufficiency planning, establish independence, and housing skills - Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning. - Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.

Provides services to help prevent families from becoming homeless, including resource referrals and eviction prevention assistance. Families are required to participate in three months of stabilization case management in order to receive financial support.

Services provided: - Advocacy with landlords - Eviction prevention and referrals to relevant agencies for those facing eviction or a utility shut-off - Helps with the Energy Assistance Program application. Provides financial assistance to young adults and families with youth who are in need of past due rent or utility payment assistance to avoid eviction and prevent homelessness NOTE: Currently out of funding as of March 2025

Agriculture: Works directly with the beginning farmers and build relations between them, local, and regional markets Asset Building: Assists individuals to achieve long-term economic goals through asset building. Helps establish finacial literacy, identifying edication and training programs. Assists in developing budgets and savings plans for home buyers. Emergency Housing Services: Assists immigrants and refugees to secure affordable, safe rental housing. Participants in this program are assisted with housing placement, a small short-term rental subsidy (based on income eligibility) and intensive case management to help families overcome barriers to self-sufficiency and obtain sustainable housing. Homeownership Counseling: Provides one-on-one counseling and group education workshops to educate individuals on the home buying process and determine whether they are ready for homeownership. Individual counseling sessions include housing needs assessment, financial assessment, credit review, and implementation of an action plan to reach each participant's housing goals. Refugee IDA Program: An one-to-one match program to help refugees save money towards either college, car, home and owning their business. The program provides access to financial trainings, workshops, resources. Southeast Asian Farmers Education: Helps Hmong, Burmese, Karen, and Bhutanese farmers learn how to best grow their farm businesses. Program provides the resources, training, and technical assistance to help beginning and seasoned farmers to improve their income. Small Business Loans: Offers a variety of loansto economically disadvantaged clients from the immigrant and refugee communities who are intrested in starting or expanding a business. The program is designed to help start and/or expand micro-enterprises and transition towards the economic independence, business development, job creation, and community revitalization.

Provides basic needs such as hot meals, grab and go foods, clothing, laundry facilities, a shower, and more. Additionally, gives access to case managers on-site to help connect youth to education, employment opportunities, health and wellness programs, stable housing, and more. Also provides a 12-unit transitional housing facility in Coon Rapids that offers supportive services to young people who are at risk of or experiencing homelessness.

Provides assistance and support to veterans?and their dependents in obtaining benefits through state and federal programs including: - Burial benefits - Compensation - Crisis assistance - Education and training - Home loans - Homelessness prevention and intervention - Life insurance - Military pension - Survivor benefits - Vocational rehabilitation

Provides rapid rehousing, scattered site housing, and supportive housing services for youth in the Twin Cities experiencing homelessness. Units are located at Nicollet Square Apartments, Downtown View Apartments. Scattered site housing includes In Reach, Hearth, and Pathways.
