  • Loans a range of home medical equipment including wheelchairs, walkers, power chairs, canes, transfer benches, raised toilet seats, and more. Equipment are available on a first come first served basis and will hold equipment for a 24-hour period.
  • Swap Shop: A self-directed equipment listing available for persons shopping/looking for used equipment.

Peer Support:
  • Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals
  • Match interested consumers with a peer mentor who provides support, problem-solving, social skills, and more
Personal Assistance:
  • Offer skills instruction curriculum for individuals living with a disability the fundamentals of managing their personal attendant care including how to hire, manage, and more
Recreational Groups:
  • Holds various events for individuals with a disability or a health condition to meet and socialize with others through dances, parties, picnics, and outings
Staying and Aging in Place/Accessibility Assistance:
  • Offers accessible modification assistance and accessibility assessments for homes and businesses for safe access and increased independence in people's homes and businesses
Support Groups:
  • Various support groups are available to individuals who want learn more about their particular disability, wanting peer support, or first-hand information on how to manage their disability. Some support groups are directly coordinated by Options and others are independent.
Youth Transition Services:
  • Assist youth ages 14 - 22 years who are transitioning from high school into the next phase of their life with learning the skills necessary and acquiring the resources needed to be successful

Drop-in center for homeless youth: ?- Housing Hub: Connects youth experiencing homelessness or at risk of being homeless with a Housing Specialist to provide housing problem solving. Housing Specialists can assist the youth with practical solutions, identify sources, alternatives, and mediate conflict resolution between friends or family. The initial access point to be connected with a Housing Specialist is short-term with a 30-day timeframe. After 30 days initial contact, households with no other pathways identified will be connected to a Housing Assessor and placed on the Housing Registry.
  • Provides computers for homework and job searches, a game room, kitchen, lockers, showers, storage areas, and a washer and dryer
  • Staff help youth get help with employment and housing searches, connect to resources, and receive basic need items (clean clothes, food, sleeping bags, and toiletries)

Provides homeless outreach and case management services to adults with serious and persistent mental illness. Offers housing options including crisis beds, rental assistance, and permanent housing. Outreach, case management, and rental assistance are also available to homeless youth under the age of 24. NOTE: Single adults and youth must get a pre-screen through United Way 211 to access this program. The pre-screen information is provided to Saint Louis County Housing Assessment Team (SLCHAT).

Offers person-centered support services to individuals living with a disability or mental health condition. Services provided:
  • Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless
  • Develop life skills including self-sufficiency planning, establish independence, and housing skills
  • Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning.
  • Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.

  • 24-Hour Responder Service: On-call staff help with health and personal injuries. The service is often paired with Independent Home Support with Training program. Staff will respond by phone or in person depending on the level of services.
  • Acute Behavioral Services for Children and Adults: Services provided to individuals with interfering behaviors and/or anti-social issues, who have a history of law enforcement involvement, have difficulty living with others and require high staffing needs, are in frequent crisis situations, or coming out of an institution.
  • Case Management: Available to residents of Hennepin, Ramsey, and Wright Counties who receive funding through various waivers and/or programs. Assists navigating the social service system and provides information and advocacy.
  • Integrated Community Supports: Provides support and training/habilitation in community living service categories to adults ages 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person's living unit or in the community.
  • Individualized Home Supports: Provides a blend of in-person and remote services that focus on support and training of a person who needs to maintain or increase independence for their continued experience living in the community. Also offers services to relatives working to support the person's goals.
  • Residential Services: Operates residences for adults with significant behavioral, medical, or physical challenges including brain injuries. Residential group home programs are licensed as adult or child foster care, or a supervised living facility. Each site serves 2 - 6 people.
  • Supportive Employment Services: Provides individuals with intellectual and developmental disabilities assistance with initial stages of the employment assessment period, job search and development, job coaching, and working independently
  • Vocational Rehabilitation Services: Offers a variety of services including job-seeking skills training, job coaching, social coaching, post-secondary education, and more. Also, supports student youth as they search for and find competitive employment and helps them explore post-secondary education.
