Homelessness Prevention Programs
Provides triage services for situations as needed and reviews county case information. Services include:
- Assistance with housing and basic needs
- Community Collaborative Fund: Initial assessment for households at risk of homelessness. Individuals must have been denied by the Saint Louis County EGA or EA program, or have already used EGA/EA within the last 18 months. ESG (Emergency Solutions Grant) funds are determined to be used for rent, rental deposit, mortgage, or utilities. NOTE: There is currently no funding at this time.
- Forms and certificate assistance for county programs such as SNAP, WIC, EGA/EA, GA, child care assistance, MFIP, Minnesota Supplemental Aid, or Housing Support. Also, follow up with issues regarding MNsure applications.
- Referrals to community services, mental health, and chemical dependency services outside of Saint Louis County

Primary services include coordinated entry access point for individuals who are homeless or at risk of being homeless. Case management is offered to prevent future issues with homelessness. Financial assistance may be available but is NOT provided in all situations, requirements vary by programs and funding.
Potential assistance consists of:
- Funding for housing and utility costs/deposits
- Funding for transportation costs via vehicle repair or public transit
- Clothing and food assistance as well as other basic needs
- Housing stability case management
Programs may include:
- Limited fund for short term hotel stays in tandem with case management services
- Long-term homelessness supportive services in Aitkin, Carlton, Isanti, Mille Lacs, Kanabec, Pine and Chisago County to help with housing search and placement for families and unaccompanied youth
- Assistance for homeless ex-offenders in Chisago, Isanti, Kanabec, Mille Lacs, and Pine County, housing placement as well as rent and deposit funds may be available
-Transitional housing services for homeless youth ages 16 - 22, as well as some financial assistance for rent

- Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence in people's homes and businesses
Assistive Technology Network
- Assist individuals in the process of obtaining assistive technology devices
- On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed, the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited
Homemaking Services
- Assist individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping

- Up-to-date disability information and referrals to various agencies and individuals within the region
Independent Living Skills training

- Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals
- Match interested consumers with a peer mentor who provides support, provide problem-solving solutions, social skills, etc.

- Provide information, resources, and materials on accessibility guide, ADA, disability laws, etc.
Respite Care Services
- Provide short-term in-home care and support when the primary caregiver or family members need to be absent from the home.

Transition/Relocation Services
- Assist individuals living with a disability in transition from nursing facilities and other institutions into community settings with the goal of preventing institutionalization
