Offers volunteer drivers who provide safe, affordable, and flexible supplemental transportation for worship services, medical appointments, access to Metro Transit, shopping local businesses (grocery stores, food shelves), community services (volunteer or social events), and running errands.Volunteer drivers use their personal vehicles and have passed a background check. Volunteer opportunities include driver, dispatcher, sponsor, and rider registrar.

Accessible transportation is provided for activities, medical appointments, dental appointments, employment, and senior meal sites.

Provides the following services: - 24-hour and holiday availability- Companion services- Errand running/shopping assistance- Homemaker assistance- In-home meal preparation- Mobility assistance, transfer with gait belt, TED Hose Assistance- Personal care such as dressing, bathing, grooming, and hair care- Respite care- Transportation to medical appointments, grocery, etc. to clients who require a minimum of 3 hours of home management and home care a week. Clients must be able to transfer to the vehicle. Door-to-door service provided. Driver will wait for the client during the appointment.

Home CareSpecialized nursing care is provided by public health nurses in the home to assist with medication management, wound care, dressing changes, post-operative care, and other services ordered by the client's physician. The public health nurse may also assign a licensed practical nurse or home health aide to assist in the care of the client.Home health aides/elder care aides provide personal care (hair washing, showers, nail care, etc.), visiting, light housekeeping, and other services for clients with medical or health problems. Aides may also monitor vital signs and provide basic health services. The public health staff is available to perform home safety checks for families with children or for elders. A limited number of safety items may be available following the home safety check. Instruction on how to use this equipment will be provided, although the nursing staff cannot install the equipment for families.Community Health Representatives (CHR) TransportationProvides transportation for individuals to medical, dental, and optical appointments only after all other resources are exhausted. Appointments should be confirmed with the CHR's schedule to assure that clients arrive on time to their appointments. No one under the age of 18 will be transported without a parent. All persons being transported are required to wear a seat belt at all times. Delivers medical prescriptions to homebound elders who have no other means of obtaining their medication.

Provides transportation for area Vets to VA Hospital or clinic in Minneapolis or St. Cloud. Rides scheduled in advance on a first come, first serve basis. Wives of Vets may ride along when room exists and utilize van to visit husbands at the VAMC. Printed schedules can be obtained from Veterans Services at 218-327-2858. Also available at VFW and American Legion Posts.

Tribal Assistance provides financial assistance, up to a maximum of $300 annually for elders and individuals with a disability on a case-by-case basis.Elder Advocates assist with transportation, food shelf visits, grocery shopping, delivery of commodities, medical appointments, and medication pick-up, completion of forms and paperwork (county, social security, health insurance). Will transport and accompany elders/disabled individuals to appointments pending availability of van and staff.

Eligible riders can receive up to 6 rides per calendar year.MedKab: A wheelchair-accessible transportation service for M Health Fairview patients who cannot use public transport. The service is available for appointments at the University of Minnesota Medical Center and other M Health Fairview facilities.

Errands and shopping assistance- Volunteers will run local errands like picking up prescriptions or getting groceries for homebound seniors ensuring that they have the needed supplies to maintain their health and continue functioning independently at home Friendly visits- Volunteers provide social interaction for those who live alone or who would benefit from additional support and companionship Handyworker and yardwork assistance- Volunteers provide assistance with minor home repairs and maintenance (recipients are expected to pay for needed materials), outdoor chores such as garden projects, yard work, cleaning out gutters or snow removal Housekeeping and chore service- Volunteers provide assistance with routine cleaning and/or other occasional deep cleaning around the home to help ensure that seniors have a clean and safe environment which helps reduce safety risks that would be present if the senior attempted to do these chores on their own Resource and referral- Provides resources and referrals for any number of services that are not provided directly by agency Respite care- Volunteers provide the primary caregiver a temporary reprieve from their caregiving responsibilities and provide companionship and support to the senior receiving care. Faith In Action volunteers are not able to help with personal care such as toileting/bathing or assist with transfers or provide medical services such as administering medication or changing dressings. Transportation- Volunteer Drivers transport and escort participants to medical and non medical appointments helping increase mobility for seniors who are unable to drive or providing a safe alternative for those who still have a vehicle and license. Mileage reimbursement is available to all volunteer drivers.

Provides a shared ride program offering transportation to anyone who needs a ride. Uses public transit buses and volunteer drivers who use personal vehicles. Transportation is for individuals with mobility issues who use walkers, standard, extra-wide, geriatric, and electric wheelchairs, car seats, and scooters. Also able to transport medical equipment. Travels to all Twin Cities and Rochester medical facilities and airports. Accompaniment travelers are welcome.
