Tribal Assistance provides financial assistance, up to a maximum of $300 annually for elders and individuals with a disability on a case-by-case basis. Elder Advocates assist with transportation, food shelf visits, grocery shopping, delivery of commodities, medical appointments, and medication pick-up, completion of forms and paperwork (county, social security, health insurance). Will transport and accompany elders/disabled individuals to appointments pending availability of van and staff.

Services include: - Alzheimer's and Dementia Care - Chronic Illness Care - Companionship - Home Helper: Cooking, grocery list preparation/shopping, light housekeeping, and meal planning - Hospice Support: Home care, medication management, personal care, and respite care - Nurse Directed Care: Catheter care, incontinence care, medication administration, oxygen therapy, and vital sign tracking - Personal Care: Bathing/bathroom assistance, dressing, grooming, and mobility assistance - Transportation: Providing rides to various appointments and activities

Provides transportation to those who need reliable transportation to access essential services such as work, school, and medical care. Assistance may include bus passes, gas cards, and other limited support options.

Volunteers drive their own vehicles to take people to medical and non-medical appointments, run errands, and visit their loved ones. Volunteers can also drive the Faith in Action van which has a lift system and can accommodate 1 or 2 wheelchairs and a caregiver or two.

Assisted Transportation is a program that consists of volunteer drivers who provide transportation to/from medical appointments anywhere in Minnesota for individuals 60+. Client contributions are encouraged, but not required.

- Case management and referrals for programs such as Meals on Wheels, social services - Door-to-door transportation mostly for health related appointments or for hair salons/barbershop appointments. Clients must be able to transfer into the car on their own. Escort services may also be available, but these requests must be made far in advance. - Homemaking, yard work and home maintenance services - Nursing care - Personal care - Senior exercise classes are offered daily; transportation available. - Social outings - Speakers on senior health topics - Support for caregivers of seniors who live in the service area, and for caregivers who live in the service area even if the cared-for senior lives elsewhere, including out of state.

Non-professional services provided: - Errand running and shopping assistance - Friendly visiting - Light housekeeping - Meal preparation - Mail management and personal correspondence - Repairs and minor maintenance of home and yard, including snow removal. There is insurance coverage for volunteers during the times they are providing CREST services.?? - Telephone reassurance - Transportation is available for appointments, church, social engagements, and shopping. Request for transportation must be done 2 days in advance and are not wheelchair accessible. Both curb-to-curb and door-to-door service is available depending on need. No fees and donations are accepted. - Volunteers do not give medications, do not assist in transferring sitting to bed, etc.

The class provides healthy recipes, food preparation, and more. Availability is limited. - Volunteer Rides: Coordinates rides that are not available through other services for local and long distance medical appointments, meetings, errands, and social events.
