Sexual Assault Education, Therapy, and Crisis Services - Canvas Health

Provides therapy, crisis services, and education to victims of sexual assault and teens through a range of services: - Telephone crisis response - Short-term counseling - Responds to local hospital emergency rooms - Legal advocacy - Safe at Home application assistance for address confidentiality - Support group for individuals who have experienced sexual abuse designed to help people begin to recover from the effects of trauma and to help them find ways to grow, flourish and enjoy healthy relationships and happier lives. Individuals receive support from peers and advocates. Individuals learn about dynamics and the culture surrounding sexual violence, common reactions and feelings, as well as coping skills that may aid in the healing process. - Works with victims of sex trafficking and sexual exploitation - Workshops and Education: Offers community education speakers on a range of topics such as bullying, cyberbullying, sexting, healthy relationships, impact of domestic violence on child witnesses, media violence, sexual assault awareness and prevention, and teen dating violence

The program's purpose is to support and empower older adults and caregivers by addressing their individual needs and preferences, enabling them to live safely and independently in their own home, for as long as possible. - Supplemental Nutrition Assistance Program (SNAP) Applications: Provides assistance with applications for SNAP (Supplemental Nutrition Assistance Program) a county-run, state-supervised Federal program that helps Minnesotans with low incomes get the food they need for sound nutrition and well-balanced meals. The program issues electronic food support benefits that can help stretch your household food budget. - Information and Referral: Offers information on resources of local food shelves - Nutrition Assistance Program for Seniors (NAPS) Applications: The program is designed to provide healthy and nutritious food products each month at no cost to eligible individuals over the age of 60. Helps eligible individuals apply for the program. - Second Harvest Heartland: Provides a free food distribution program to those in need - Prairie Five Meals Program: Offers congregate dining, home delivered, and frozen meals serving 34 communities in the area. Frozen meals are available for weekend, holiday, emergency and rural delivery. The program promotes health and independent living in the senior population through improved nutrition and reduction of social isolation.

Housing and Community Support Programs - Rise, Inc.

Offers person-centered support services to individuals living with a disability or mental health condition. Services provided: - Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless - Develop life skills including self-sufficiency planning, establish independence, and housing skills - Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning. - Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.

Services include: - Food Assistance: Congregate dining locations offering low-cost meals, low-cost frozen meals, home-delivered meals, NAPS, and SNAP - Healthy Living: Blood pressure checks, fall prevention screenings, telehealth conferences, Tai Chi and yoga sessions, and stretching/balance education - Information Services: Offers memory loss education and Dementia Friends, Medicare counseling and assistance, fraud/scam identification and prevention, assistance with internet search, assistance with Healthcare Directives and Power of Attorney forms, and classes to older adults - Mobile Community Center: Brings services and socialization to the door or community. Northern Area Triage Team - Saint Louis County Public Health and Human Services

Provides triage services for situations as needed and reviews county case information. Services include: - Assistance with housing and basic needs - Community Collaborative Fund: Initial assessment for households at risk of homelessness. Individuals must have been denied by the Saint Louis County EGA or EA program, or have already used EGA/EA within the last 18 months. ESG (Emergency Solutions Grant) funds are determined to be used for rent, rental deposit, mortgage, or utilities. NOTE: There is currently no funding at this time. - Forms and certificate assistance for county programs such as SNAP, WIC, EGA/EA, GA, child care assistance, MFIP, Minnesota Supplemental Aid, or Housing Support. Also, follow up with issues regarding MNsure applications. - Referrals to community services, mental health, and chemical dependency services outside of Saint Louis County
