Helps eligible individuals apply for the program. - Second Harvest Heartland: Provides a free food distribution program to those in need - Prairie Five Meals Program: Offers congregate dining, home delivered, and frozen meals serving 34 communities in the area. Frozen meals are available for weekend, holiday, emergency and rural delivery. The program promotes health and independent living in the senior population through improved nutrition and reduction of social isolation.

- A food shelf open to participants to walk-through and shop for food once per week. ARHMS - Adult Rehabilitative Mental Health Services: ARHMS clients meet in person with their provider once a week or every two weeks for 1 - 6 hours minimum, depending on client need. Provides a range of services that helps an individual develop and enhance psychiatric stability, which include skill building in the areas of: - Basic social, living, and parenting skills building - Budgeting/money management - Crisis assistance (as needed) and develop relapse prevention skills - Coping skills education - Employment and/or education related skills (GED/Resume building/anxiety reduction) - Healthy lifestyles and household management - Interpersonal communication/boundary building TCM - Targeted Case Management: TCM clients meet with their provider 1 - 2 times a month and can coordinate services by phone or telehealth. Provides a range of services that help the client remain in the community independently and assist clients to: - Access social service agencies (make referrals, complete paperwork for programs) - Assistance in applying for financial resources - Maintain housing benefits and/or find new housing Transportation set up and coordination

Helps individuals access needed medicines at an affordable cost in the dosages needed to maintain their health by enrolling in patient assistance programs for which they may be eligible.
Provides application assistance for a variety of programs such as: - MN Navigator Services: Helps individuals and families apply for health insurance enrollment, navigate health insurance options, and compare different health plans, including Medical Assistance or MinnesotaCare. The navigator will help clients choose and enroll in the right program for them. - Supplemental Nutrition Assistance Program (SNAP): Helps individuals apply for the SNAP program. Provides education and support on the application process to qualifying low-income individuals and families.
Provides the following services: - Financial assistance with rent, utilities, rent and utilities arrears, transportation, basic needs, food, application fees, time-limited housing subsidies, emergency shelter, household necessities, etc. NOTE: Financial assistance may vary and is not guaranteed for every applicant. - Information and coordination related to public benefits, veterans' benefits, housing stability, employment, and legal issues In-House Services: Housing stability case management, rental subsidies, legal, employment, justice-involved coordination, healthcare navigation, landlord engagement, outreach, documentation specialist, financial coaching, and free voluntary representative payee services
