Provides services including: - Behavioral health: outpatient therapy, psychological testing, presurgical mental health screenings, and evaluations for learning disorders - Family medicine: comprehensive health and wellness care for all ages including routine check-ups, minor illnesses and injuries, and other health concerns - Heart and vascular care - Nutrition counseling

Case managers work directly with families and those with intellectual and physical disabilities to meet their health and safety needs - Case management - Referral to service providers and community resources - Screening, assessment, consultation - Waiver screening for eligible individuals
