Assistive Technology Equipment Loan
Provides specialized inpatient and outpatient services to Veterans, including:
Care Program and Support: Provides caregiver support to Veterans and their families at home and in the community, including a Caregiver Support hotline, comprehensive assistance for family caregivers, adult day health care centers, home-based primary care, home hospice care, homemaker and home health aides, home telehealth, remote monitoring care, respite care, and skilled home care. Community Living Center: Provides transitional care including skilled nursing care, for complex medical problems requiring rehabilitation and medical management, pharmaceutical and laboratory service, intravenous therapies, radiation therapy, rehabilitation services, end-of-life care, respite care, and geriatric evaluation and management. Geriatric Research Education and Clinical Center (GRECC): Provides a multidisciplinary program designed to address the disorders of the aging nervous system for elderly Veterans. Room 4L-100, (612)467-2051, 7:30 am - 4 pm Monday - Friday.
Home and Community Care: Provides and coordinates many services through a continuum of care for Veterans in their homes and community settings, including Home-Based Primary Care, Medical Foster Home Program, and providing programs and services that are delivered by community partners.

Services can be delivered in the home, a VA facility, community setting, or at a Blind Rehabilitation Center, and include: adjustment to blindness counseling, patient and family education, support groups, benefits analysis, short term and comprehensive residential training (VISOR Program and HOPTEL and Non-HOPTEL Program), outpatient rehabilitation services, and the provision of prosthetic aids. Home and Community Care: Provides and coordinates many services through a continuum of care for Veterans in their homes and community settings, including Home-Based Primary Care, Medical Foster Home Program, and providing programs and services that are delivered by community partners.
