Evidence-Based Health Education and Support - Southeastern Minnesota Area Agency on Aging - Rochester

Arthritis Foundation Exercise Program: Exercise, nutrition and health education, fall prevention, and medication management program for older adults in multiple locations around the region. Leaders are certified Arthritis Foundation instructors. Living Well with Chronic Conditions: A 6-session class to help people cope with chronic conditions; lead by 2 trained leaders. Various locations and trainers available through out the region. Matter of Balance: A class to learn about falls prevention that meets 8 times and is lead by 2 trainers, one whom is a volunteer. PEARLS: A community-based treatment program for older adults with depression. A PEARLS coach will meet with the person for eight, one hour sessions that are customized to support them and their needs. Learn how to problem solve and change their behavior to support their well-being. Powerful Tools for Caregivers: A 6 week class for caregivers to enable them to provide care for a loved one longer and with more confidence and control. Class is lead by 2 trainers and is available in various locations throughout the year. Tai Ji Quan: Moving for Better Balance helps older adults improve their balance and reduce the likelihood of falling. Research has shown that participants are half as likely to fall and are less fearful of falling. Participants typically have improved physical health that leads to better functioning in daily activities, improved balance, muscle strength, flexibility and mobility, and better mental health.Classes beginning in some areas in 2015.

Services offered: - Addiction Medicine Clinic: Medication-assisted therapy, medication management, tobacco cessation, and in-person or virtual visits - Adolescent Residential: Offers chemical dependency treatment, access to mental health services, a school program, and family therapy. The program is 6 - 8 weeks in length. - Comprehensive Assessment: Chemical dependency evaluations. Same-day assessments are often available. - Evening Outpatient: Primary substance use program that offers a mental health component. The program runs for 6 weeks and requires two 3-hour sessions per week.

In-Home Assistance - Neighborhood Network for Seniors

Provides a wide variety of free and low-cost home health care services. Services provided: - Blood pressure checks from an outreach nurse - Community education seminars designed for caregivers, seniors, family members, and the public on a variety of topics for older adults - Computer/technology help - Friendly visits from volunteers to help with transportation, chore support, grocery shopping assistance, meal preparation, plant care, pet care, telephone reassurance, or offer emotional support. Services are based upon volunteers and their availability to offer services. - Home safety checks to meet with older adults and discuss their needs, develop a care plan, and refer to professional services - Home chore assistance including seasonal yard work and occasional snow shoveling. Yard work may include spring and fall cleanup, installation of window AC's, and help around the house. - In-home foot care for non-diabetic individuals for a fee - In-home health assessment for a free in-home nurse visit to discuss health concerns and goals - Rides to medical appointments with at least 1-2 weeks' notice before an appointment
