Special Access Program - Korean Service Center

Programs providing assistance to non-English speaking Korean elderly to access community and government services that are not easily accessible due to language and cultural barriers. Services including: - Citizenship service: Citizenship application, passport, and permanent residency renewal - Consumer service: Bank, garden, and voting - Government benefits service: MSA (Minnesota Supplemental Aid), SSA (Social Security Administration), SNAP (Supplemental Nutrition Assistance Program), and SSI (Supplemental Security Income) - Health care service: Medicaid, Medicare and MNSure - Housing services: Minneapolis Public Housing application and renewal - Language service: Interpretation and translation - Social and Cultural service: Korean holidays, major holidays and events, and parent's day - Supportive service: Education service and support groups

Services include: - Community Supports: Connects people with community supports, such as home health care, meal preparation/delivery, transportation, and hiring someone to help at home or with yard work - Health Insurance Counseling: Helps people with questions about Medicare and other health care questions - Housing Resources: Helps people understand their housing options, such as assisted living requirements, comparing housing services and costs, and more - Preadmission Screening (PAS): Manages preadmission screening (PAS) for anyone planning to be admitted into a Minnesota nursing home - Prescription Drug Assistance: Helps those who qualify find ways to reduce prescription drug costs, including finding manufacturers' patient assistance programs and completing applications - Website of Resources: Offers a website to locate resources throughout the state for help with the home, medical costs, tax preparation, legal/advocacy, housing, transportation, food programs, benefits, health care, mental health, and more. Patient Support Helpline ZERO360 - ZERO Prostate Cancer

Provides a secure online portal and a helpline free of charge to support individuals navigating their prostate cancer journey. Services include: - Assistance with navigating insurance coverage options of patients and helping resolve issues with their insurance company. Secure access to care if individuals are uninsured. - Ensure access to care and benefits such as Social Security Disability (SSI or SSDI) or state disability benefits, employer-sponsored benefits, sick leave, short-term and long-term disability, and COBRA. - Finding resources for financial aid and/or prescription drug assistance programs - Help with applying for programs that pay for basic needs expenses - Secure online portal to connect with a ZERO360 case manager. Case managers can work with individuals to help negotiate free or reduced-cost treatment or set up payment plans.
