NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services. The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs; includes assessing what level of service is needed for care such as homemaker, home health aide, adult day care.

• Computer classes

• Foot care clinic

• Health information lectures

• Medicare health insurance counseling

• Woodworking instruction

• Information about public programs that can help pay for programs
• Pre-admission screenings prior to nursing home admission
• Transition assistance for people who want to return to community settings after admission to a facility
