The assessment determines eligibility for publicly funded programs and services including: Alternative Care Program, Brain Injury Waiver, Community Alternative Care Waiver, Community Alternatives for Disabled Individuals Waiver, Developmental Disability Waiver, Elderly Waiver, Case management for people with developmental disabilities, and Semi-Independent Living Services.
