Store-and-Forward Telehealth: For Veterans with known diabetes to prevent eye diseases caused by diabetes. Common services provided are retinal imaging and dermatology. Clinical Video Telehealth: Provides a live video connection with a specialist at another location and addresses many health conditions and diseases.

Equipment Loaner Closet: 

- Loans a range of home medical equipment, including wheelchairs, walkers, power chairs, canes, transfer benches, raised toilet seats, and more. Equipment is available on a first-come, first-served basis and will hold equipment for 24 hours.

- Swap Shop: A self-directed equipment listing available for persons shopping/looking for used equipment.

Information and Referral:

- Up-to-date disability information and referrals to various agencies and individuals within the region

- Other Services: Includes disability benefits application assistance, housing assistance, emergency disaster preparedness plan, and more

Nursing Facility Transition:

- Assist individuals living with a disability to identify in transition from nursing facilities and other institutions into community settings to prevent institutionalization

Peer Support:

- Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals

- Match interested consumers with a peer mentor who provides support, problem-solving, social skills, and more

Personal Assistance:

- Offer skills instruction curriculum for individuals living with a disability, the fundamentals of managing their personal attendant care, including how to hire, manage, and more

Recreational Groups:

- Holds various events for individuals with a disability or a health condition to meet and socialize with others through dances, parties, picnics, and outings

Staying and Aging in Place/Accessibility Assistance:

- Offers accessible modification assistance and accessibility assessments for homes and businesses for safe access and increased independence in people's homes and businesses

Support Groups:

- Various support groups are available to individuals who want to learn more about their particular disability, want peer support, or first-hand information on how to manage their disability. Some support groups are directly coordinated by Options, and others are independent.

Youth Transition Services:

- Assist youth ages 14 - 22 years who are transitioning from high school into the next phase of their life with learning the skills necessary and acquiring the resources needed to be successful

Resource center assists with the following services:

- Advocacy: Includes areas of housing, civil and human rights, employment, transportation, and others. The Center advocates for state, local, and federal legislation.

- Aging in Place Program: Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence.

- Assistive Technology Network: Assists individuals in the process of obtaining assistive technology devices.

- Homemaking Services: Assists individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping.

- Independent living skills training

- Information and Referral: Provides up-to-date disability information and referrals to various agencies and individuals within the region.

- On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed; the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited.

- Peer Mentoring: Assists individuals living with a disability to identify goals and create a unique plan to achieve those goals. Matches interested consumers with a peer mentor who provides support, problem-solving solutions, social skills, etc.

- Public Education: Provides information, resources, and materials on accessibility guides, ADA, disability laws, etc.

- Respite Care Services: Provides short-term in-home care and support when the primary caregiver or family members need to be absent from the home.

- Systems Advocacy: Works to implement changes where needed at a broader systemic level.

- Transition/Relocation Services: Assists individuals living with a disability in transitioning from nursing facilities and other institutions into community settings, with the goal of preventing institutionalization.

The MNSure Assister Directory is an online tool that helps Minnesota residents locate nearby insurance brokers and MNSure navigators. Individuals can search by organization name, county, zip code, and even languages spoken to find an assister who can help get through the application process. Assisters can help consumers in person, by phone, or through virtual meetings. - Navigators help individuals and families apply and enroll in health plans through the MNSure marketplace - Navigators are obligated to help any consumer who contacts them for assistance - Navigators are formally contracted partners with MNSure and receive online training, pass a certification exam, and undergo a background check - Navigators can assist eligible individuals with applying for the Insulin Assistance Program - Navigators are not insurance agents or brokers
