The Center advocates on state, local, and federal legislation Aging in Place Program - Offers accessible modification assistance, ramp installation, and accessibility assessments for homes and businesses for safe access and increased independence in people's homes and businesses Assistive Technology Network - Assist individuals in the process of obtaining assistive technology devices - On-Demand Lending Library: Program will purchase the requested device and the individual can use it for as long as needed, the program retains ownership of the item. Eligibility requirements: first come, first served basis; funds are limited Homemaking Services - Assist individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping Information and Referral - Up-to-date disability information and referrals to various agencies and individuals within the region Independent Living Skills training Peer Mentoring - Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals - Match interested consumers with a peer mentor who provides support, provide problem-solving solutions, social skills, etc. Public Education - Provide information, resources, and materials on accessibility guide, ADA, disability laws, etc. Respite Care Services - Provide short-term in-home care and support when the primary caregiver or family members need to be absent from the home. Systems Advocacy (to implement changes where needed) Transition/Relocation Services - Assist individuals living with a disability in transition from nursing facilities and other institutions into community settings with the goal of preventing institutionalization

- Patients and caregivers are instructed by medical professionals on the safe use of state-of-the-art technology - Written instructions are left with caregivers for all clinical equipent Staff includes: - Registered Respiratory Therapists - Certified Respirator Therapy Technicians in some locations - Billing staff qualified to bill Medicare, MA and private insurance - Equipment technicians trained in all equipment - Medical services coordinators experienced in obtaining prescriptions and setting up service schedules - Nursing services, personal care and chore service available in some locations - Medical equipment and supplies delivered directly to homes, nursing homes and hospitals within service area - Staff is available for emergency service 24 hours a day
