Home Health Care Medicare certified home health agency available to provide: - Skilled Nursing services - Home health aide personal care services - Physical therapy in the home - Occupational Therapy - Speech Therapy - Senior companions - Respite care - Care at home during recovery from an acute illness or injury - Care at home for chronically ill/disabled adults and children - Care at home for terminally ill - Help adjusting to an illness or disability - Information about a new diet, medication or disease - Personal care & support services to help stay in the home as long as possible - Information about other community resources - Access to medical supplies & equipment - Long Term Care Consultation - Prior to nursing home placement, an assessment is done by a Public Health Nurse and Social Worker - Relocation services back into community setting

Eligibility requirements: first come, first served basis; funds are limited Homemaking Services - Assist individuals in managing household activities including housekeeping, laundry, meal preparation, and shopping Information and Referral - Up-to-date disability information and referrals to various agencies and individuals within the region Independent Living Skills training Peer Mentoring - Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals - Match interested consumers with a peer mentor who provides support, provide problem-solving solutions, social skills, etc. Public Education - Provide information, resources, and materials on accessibility guide, ADA, disability laws, etc. Respite Care Services - Provide short-term in-home care and support when the primary caregiver or family members need to be absent from the home. Systems Advocacy (to implement changes where needed) Transition/Relocation Services - Assist individuals living with a disability in transition from nursing facilities and other institutions into community settings with the goal of preventing institutionalization
