Transitional Care - Catholic Eldercare
Assists individuals transitioning home from a hospital stay. Transitional care supports the healing process and can improve health outcomes. A team works with an individual, their physician, social workers, and the hospital to ensure each individual gets the care they need to recover quickly, safely, and effectively. Transitional care may include rehabilitative therapies such as physical, occupational, and speech therapies as needed.
