Provides supportive services to adults who have a serious and persistent mental illness in efforts to have them remain in their own community and able to live as independently as possible. - Providing outreach services, such as home visits and wellness checks - Assisting consumers with: - Problem-solving and connecting with mental health and other resources - Accessing community resources and supports - Applying for health insurance and other benefits - Developing a healthy lifestyle and independent living skills, such as: - Cooking and nutrition - Household management - Budgeting - Shopping - Accessing community resources and supports - Taking medications as prescribed - Consistently attending appointments - Developing interpersonal communication and social skills - Educating themselves about mental illness, treatments, and recovery - Developing relapse prevention and symptom management skills
